Has anyone used the BW formulas that were selected by a practitioner who used muscle testing to determine the most appropriate formula?
I feel this would be the most effective way to use them.

In my opinion, muscle testing is irrelevant. Your doctor should go by clinical symptoms and test results.

I was originally prescribed BW formulas based on clinical diagnosis, but have since been taking them daily by muscle testing them to see which formulas will work for me that day. It's fascinating to see what comes up. I have lots of formulas to choose from.
I have used a practitioner before to muscle test the formulas on me, but the body changes daily. What pops up one day, may not pop up the next (happens all the time with me). It is easier for me to do it myself and IMO, really accurate. If you're not muscle testing, you're guessing what to hit, and probably just wasting it (just my opinion).
Clinical symptoms are helpful in choosing formulas, but IMO muscle testing is the single most valuable thing you can do for yourself. Even my pcp told me that it's the most accurate tool we have. I learned how to do it off the internet (google). There are several different methods to choose from. I muscle test everything now.
