I've already been Dx in 1999 and 2000 and years following them. I'm want to go all natural as in Herbs, acupuncture, light, and healthy foods that I'm not able to prepare. etc.

Don't go if you don't want to. You're an adult.
It sounds like you don't want to make waves with your brother, but remember, YOU come first.
If you don't want a big confrontation about the whole thing, just don't fill out the paperwork.
When he tries to set a date, just say, "I'll think about it and get back to you." or "There's something else I want to check out first, I'll get back to you afterwards."
Don't give a specific day or deadline. Just say,"I don't know yet. I need more time. I'll let you know when I want to go." Be kind, yet FIRM with your tone.
Don't give into pressure. Get off the phone or change the topic if you need to. You can always say something like, "I don't feel like talking about that right now." No need to give an explanation. Repeat it til he gets it.
If he doesn't, just say you have to get off the phone.

He did this because he cares about you and thinks NIH will help.
But they won't. They have an in-house lyme study that has been going on for years and never helped us. The person who runs the study publishes with Steere, Wormser, all the bad guys.
It will be very hard for your brother to understand this. Not sure it is even worthwhile to try to explain, leaving that up to you.
But it will take a lot of energy to travel that far and it will not help you or the lyme cause. This NIH study is being done to pretend to address lyme disease issues, but it doesn't. If the govt health agencies were actually honest and trying to help, our situation would be very much better.
Don't know if you know any AIDS history, but they also brought up the rear on that disease.
