I'm on vancomycin IV and looking to add a few abx to have a very potent stack. Thinking about
- azithromycin IV or telithromycin po or clarithromycin po, - bicillin-LA IV or IM or Augmentin XR po, - metronidazole po or tinidazole po
any suggestions? I guess tomorrow I'll look at bactericidal levels and MIC data for each, and the serum concentration that can be expected to be reached with each. Anybody knows where I should start with that?

Based on your list, I think probably the most powerful combo would be to add in metronidazole -- but IV instead of po. Some docs finish up treatment after rocephin with IV flagyl -- but depending on your history and neuro issues and also babs status that could turn into either the best thing or the worst thing you have ever done. Hubby says the flagyl gets into a part of his brain that nothing else he has tried touches -- but the fallout has not been pretty.
2nd choice would probably be IV zithromax -- adding that to IV Rocephin was very helpful for hubby.
This is not medical advice, just my opinion basaed on hubby's experiences.

Vancomycin is not very common treatment but Burascanno says it's extrememly effective...you'll have to let us know.
Personally I wouldn't get too nuts on more drugs, as your liver is probably taxed already. I would just go with oral Tinidazole. It's half life is longer than Flagyl and twice a day should keep levels good. bioavailability of oral metro or tini is reportedly up to 90% so oral should do fine.

sorry to hear that scorpiogirl. IV zithro is pretty expensive (~24$/g at Costco).
why do you guys take flagyl IV given that it's bioavailability is so high? is it to decrease GI side effects and to preserve your gut flora?
I read lots of good comments about the Zith/flagyl combo. Dropped a script to the pharmacy today or both (flagyl oral though).
migs outside of BID instead of TID what advantages tini has over flagyl? flagyl is way more used, is it because it's cheaper or it offers more?

You should be hitting all forms. I have done IV vanco with flagyl in the past. (oral flagyl) Great combo! It got rid of my lyme for nearly a year.
I'm getting back on it with a 3 drug combo and going to stay on it longer. IV vanco, oral tindimax and oral biaxin.
