I have had herx like effects, that or it is getting worse. Initially I didn't have shin pain, or sole pain in my feet. It was just the skin issues first.

TBH that is not a very good combo for bart. Amox doesnt hit it, doxy perhaps only very weakly and bactrim may or may not have an impact on your specific bartonella strain.
Look into rifampin, fluoroquinolones (careful here!), and macrolides. Also you may want to look at herbal options, like Buhners co-infection book. Houttuynia can also be of help for some.

Buhner says the protocol can be done concomitant with antibiotics. I agree with the idea that the herbs and antibiotics can each help the other to work better.
I don't take the full Buhner protocol with my abx, but do use several of the herbs. I'm sure there are many others who do as well.
Rifampin is usually given as an oral, but is available for IV as well.

I got over it twice with Bactrim DS but it is different for each person.
Levaquin was very effective but it gave me tendonitis. I could not tolerate Rifampin. It took about a year the first time to recover but only about 5 months the second time.
I am in remission now.

For me the strongest abxs for Bart have been Rifampin, Flagyl, Ceftin, Omnicef
Herbs: Houttuynia by hepapro (though they have a new formula for HH2 - ugh)

If I have this right from Dr. H's presentation:
BART TREATMENT:
Combinations - Clarithromycin and Rifampin - Doxycycline and Rifampin - Metronidazole and Rifampin - SMZ/TMP and Macrolide
Hydroxychloroquine added to above regimens
My understanding is Rifampin is considered the best medication to attack Bart but it needs to be administered with another medication or else drug resistance develops.

Clarithromycin (Biaxin) or Azithromycin (intracellular antibx) have been known to help OCD rooted in infections.
Looks like you are not on/ haven't tried one of those yet.
Hang in there- my daughter has OCD driven by infection, those two antibx have helped. And Augmentin. Augmentin was a game changer a few months back for her.
Augmentin helps modulate glutamate in the brain, and excessive glutamate can also drive OCD.

I've noticed much improvement in my psych symptoms since adding a macroglide. I also seem to be dreaming again, but not with the frightening vividness as in my pre-treatment days.
Artemisinin is usually torturous for me. Not nearly so now, but I also did a week of cryptolepis before getting back to art. Could it be the macroglide?
No rifampin here yet, I'll be taking sida acuta for a little bit before adding the pharm.

Our entire family's psych symptoms from bart were eliminated within 3 weeks of starting a protocol from our llmd that included parsley drops, drainage support, and fibrin agents (lumbrokinase or nattokinase) taken twice a day, one hour before meals/meds.
For the other bart symptoms (ice pick headaches, sore soles of feet, bladder issues, etc.), we've been successful with Beyond Balance Bar-2 (where rifampin failed miserably).
We were also on pulsed liposomal artemisinin. DD was on cryptolepsis for a couple of months.
Great thing about the Bar-2 is that you can titer to exactly the right dose where it hits the bart hard without hitting you too hard.

Regarding OCD:
My grandson developed anxiety and OCD. He is going through a hard time with his parents' divorce, and does not have lyme.
His symptoms were getting so bad he could barely function sometimes. He is 11 years old.
I did some research to find a natural treatment.
He is taking Inositol powder in a little juice three times daily.
It is finally helping him. He just reached a total daily dose of 10 grams about 5-6 days ago and that is when we noticed slight improvement.
He is now taking a total daily dose of about 11.5 grams and is even better. May have to increase some more.
Here is the brand we got because it contains 845 mg in 1/4 tsp. Many other brands contain less and a person would have to take many, many tabs to equal a powder dose.
He is up to 1 tsp + 1/8 tsp 3 x a day in juice.
Here is the info about the Inositol and the link to the info about how it works.
10-18 grams is supposed to be the therapeutic dose.
We increased slowly to avoid gas or diarrhea, the only noted possible side effects.

Have you looked into Lithium Orotate its a mineral. Very good to protect the nerves.
It helped with MY OCD during all of this.

Also, vitamin B6 helps, so it may be good to try both Inositol and B6.
The more bioavailable form of Vit B6 is called P5P.
I was just reading about B6 yesterday and today and I just got some for my grandson.
When I was around my grandson yesterday, he seemed about 75% improved on the Inositol.
