Many patients suffering from TBD's have auto immune issues.
On IVIG there seems to be 2 methods. 1) IVIG replacement therapy.
2) IVIG stimulation / replacement therapy (this is the therapy used with PANDAS as well and is higher does this #1 above).
In either case your blood work will have to show you have low immune system.
A normal LLMD, typically not part of an insurer network, has a difficult time obtaining approval for treatment.
Our daughter was approved for #1 by a Yale neuro as her blood work IGG and subclasses showed her immune system was not producing antibodies for normal infections.
Our daughter was approved for #2 by a different MD and she is now having these treatments.
The IVIG is one of the medications that require pre authorization from the insurer due to its cost (our daughters medication is $9k per treatment, this is only the medication, not nurse, MD, fusion center costs).
If the MD indicates Lyme autoimmune the insurer probably won't approve without separate expert concurring opinion.
