quick question, if no one knows not a bit deal, i'll ask my lawyer on monday, I'm filling out the disabilitly application and it only asked me for my diagnosis, no where to describe in detail my limitations, and it's asking me to submit the form.
Very confused, where on here do u explain all that? after you submit the form?
the only thing i see on here is a tab that says remarks, but it doesn't seem that's where you put info on what your limitations are?

shorten everything into the least words you can and yes, write it out in detail in the notes section. I know, the y don't give you much room. Ask your lawyer to be sure, but this is what I did. Applied on 8/10 and have not heard anything else.

Is this online or written application? Is this the Activities of Daily Living?
Put as much in the blanks as you can, than add the rest in remarks.
Describe as much detail in as few words a possible. (dizzy and weak when stand for 5 minutes then need to lie down, fatigue and confused after being on computer for 10 minutes, etc.)
these are examples, only. Describe your symptoms so they understand why you can't work.

The "diagnosis" is your actual medical diagnoses that are found in your medical charts. Could be chronic fatigue, fibromyalgia, lyme disease, babesiosis, hypothyroidism, depression, anxiety, rheumatoid arthritis, migraines, primary immune deficiency, seizure disorder, neuropathy, ...
You can use the remarks section to describe how these diagnoses impact your daily life and keep you from working.
If you don't feel comfortable filling out an online application, you can go and file it out in person. Or print it and fill it out with your lawyer.
This is just the first form that you will have to fill out. SSD will send you many many more. They will want to know work limitations, filled out by you and your doctor. Pain questioners. Activities of daily living questionnaires, etc.
Be honest, don't sugar coat like you would with your friends or family. Think about how you feel when you work.
You've got a long road ahead of you. Remind yourself at least you are on the right road.
(I have a condition that should have been approved automatically by Blue Book guidelines but it was not. Even with my lawyers help, I still had to go through the whole appeals process and wait for 3yrs to see the judge. I even saw 3 of SSD's own doctors that agreed with my many specialists that I was too sick and could not work.)
Try not to get discouraged. I think that is what they want you to do.

It is all about what you cannot do. Can you fix dinner? Can you go thru the day without having to stop and sleep? Are you on pain meds? Do you have another condition - like IC, arthritis, IBS, etc?
Do you have copies of your recent blood work? Xrays, scans etc?
