Many with Lyme have taken the CALDA survey already, however...
. . . they still would like to have 300 more people respond. A summary of the data collected will be presented at the upcoming IDSA Lyme Guideline Review Panel Hearing at the end of July.
Please take a few minutes to complete this survey and share this note with others who have Lyme disease.
California Lyme Disease Association (CALDA) sponsored

Results of the survey will be included in my testimony to the IDSA panel.
Speak out and be heard. Help CALDA represent your views to the IDSA at the July 30 hearing.
