Nan,
I have just the letter.

I just received denial from my private insurance co. and in there it says that even though my LLMD has diagnosed me with everything from bipolar, FM, CFS, and Lyme, based on Igenex, that I don't meet the criteria.

One of the denial reasons is that Igenex says I have lyme but not Quest and Quest is a nationally known and respected lab.

I would be more than happy to mail the latter....I could give a hoot about my personal info.....to whomever.

Let me know what to do.
