I started a new post......I can't believe my dymb brain could have missed this. I was reading Lymetoo's post on another thread about band 31 and I rechecked my Igenex results---the IGM positive are: 58, 41 and 31 IND.

So does that I truly might have Lyme if #31 was IND since it's Lyme specific??? Hot diggity dog-maybe feeling crappier might b e worth it then????

It is a potential indicator but 31 can be cross-reactive with various herpes viruses. So to be more sure, you could have your doctor order the 30/31 Confirmation test which will rule out any viral cross-reactivity.

31 can be cross reactive with herpes? Huh. I was under the impression that 31 is the most "Lyme-specific" of all bands. Ick! I was basing a lot off my 31 band.

58 - unknown but may be a heat-shock Bb protein

Band 31 is a Lyme Specific band maybe the most specific next to 39. You might need the cross reactivity test to rule out virus.

Although
the detection of antibodies against the 31- and 34-kDa
proteins may not be crucial for the diagnosis in terms of
sensitivity, these two proteins are very specific markers of
late Lyme borreliosis when they are both present.

Hi btmb03,

If you have any questions about Lyme testing, this is VERY helpful and important to know-

Basically, cross-reactivity is very VERY rare. He explains it better than I am able to right now.

When I read it the first time I had to read a paragraph or 2 every day or so, but it is very well put together, and the bolded parts are very helpful.

Please don't get too hung up on testing - you will see on your lab results (always ask for your copies of the full reports of every test!) that it says clearly "this test is not diagnostic - Lyme is a clinical diagnosis" or something to that effect,

CDC positive is for epidemiological study ONLY, not for diagnostic criteria. It will say that on your test results from the lab. If you have ANY *Lyme specific* band that means ONLY Lyme can cause that band. If it is a cross-reactive band it is not a specific band, as far as I understand it anyway.

It can't be stressed enough that it is not only possible but the most common scenario that someone with Lyme will not have a positive test result. There are many reasons for this, some of which Dr. C goes into in that link I gave you.

You need to see a Lyme specialist or LLMD, and they will be able to determine if you have Lyme or something else. They will try to exclude other conditions first if possible.

SForsgren, please clarify your previous statement regarding WB band 31, as I can't find any information to back up your claim.

As far as I have learned, band 31 could only very rarely be possibly confused with L. interrogans (32 kDa), if the test is done or read improperly.

The reason that band 31 is not included in CDC epidemiological criteria is that it is one of the bands that will appear if someone has had the old Lymerix vaccine. It is specific to Borrelia burgdorferi (Lyme), as far as I have read.

Could you please help us understand your statement?

I would appreciate more information too SForgren since I am seeing my LLMD soon. My brain is questioning why one would need a confirmation test if band 31 did NOT cross-react with something..how many of us ever had the Lyme vaccine?

Not many, so perhaps viruses? Info pleeeease!!
