I believe it means a person has an IV port inserted in a vein and the abx is dispensed interveneously for long term administration.

No it means a person takes antibiotics for several days a week, takes a break, and then gets back on them. For example, 3 days on, 4 days off. Some LLMDs recommend it, others condemn it for fear of creating superbugs.
