Did the guidelines previously mention using Plaquenil for Bartonella?
I have never heard it being used that way, though it doesn't mean it isn't.
I have only seen it reference in treatment of Lyme.
Up for more input.

Hmmm... I have only heard Biaxin or Zithromax with Plaquenil.
Are you thinking of Rifampin?

From the Guidelines, "Alternatives to levofloxacin [Levaquin] include rifampin, gentamicin and possibly streptomycin."
So, why is it I have seen frequently? on the board that Rifampin is given with Levaquin, when the Guidelines suggest Rifampin as an alternative to Levaquin?

Rifampin is never given alone. It can quickly build up resistance, so it is rx'd in combo, often with Doxy, Zith or Biaxin.
I can't say why some may have taken levaquin and rifampin together.
I have seen this quoted by a doctor:
"Except for the tetracycline antibiotic group, the fluoroquinolones are not generally used in combination with other antibiotics."
