It took me years, but I seem to have beat babs - or so we hope.
I had both microti and duncani.
Would do 6 - 8 mos of treatment, then when symptoms were gone, would go off of babs treatment only to relapse within 4 mos each time.
I've been off babs treatment since last Feb, and so far no babs symptoms have returned.
Mepron with Ketek at first, along with another abx (we varied that one quite often, I can't remember all the combos).
Then Mepron with Biaxin, then Mepron with Zith, then Malarone with Zith - each of these coupled always with another class of abx.
A Bactrim and Biaxin combo worked well also.
At various times we added Plaquenil and/or artemisinin - but I could never tolerate either of those well, so they did not last.
So - for myself - the key seemed to be length of treatment, and keeping at it.

I had babs duncani. I treated with Mepron and zithromax and artimisinin for 6 months. Then, I went on bactrim for about 5 months. I also took an herbal tincture for babs called Enula. After that, I took an herbal tincture called Mora and then one called Crypto-Plus.
I have been off all babs meds for almost 3 months now with no return of babs symptoms so far.
