it started as a blotch now it's looking like a ring... no tick (possibly) for more than a month... weird?

Not weird. A month is in the right time frame for a rash to appear.

According to the CDC an EM rash >5 cm is "confirmatory for lyme disease". If he does not have any other symptoms, a 6 week course of doxy at 200 mg BID should protect him according to the ILADS guidelines. Be sure to download those guidelines.
If he has fever, headache, stiff neck, stiff shoulder,pain in lower back or pain in hips, elbows or knees, he would need a longer course.
Read the guidelines carefully please to assure that you can advocate for adequate treatment to prevent long term problems or disability.
TAKE A PHOTO OF THE RASH...so if it goes away, you have documentation of the rash... Also go to the doctor as ask them to check it. Then it will be "medically documented". Ask for a copy of the record for your files (some doctors are notorious for loosing records such as rashes )
