It's on a patient medical record. And it's circled.

It's also context-sensitive. When I went to nursing school, NKA meant no known allergies, as in meds. NKDA meant no know DIETARY allergies, as in food. Which is why most common abbreviations are now outlawed in hospitals, and most everything must be spelled out.
