I have an appt with my ID doc this week. What co-infections do I ask him to test for? Can I tell him to send the blood work to Igenex? I have already tested Positive for Lyme. Thanks for any input.

You can print out the forms on the IGeneX website and hand them over to your doctor in the hopes that he'll actually use IGeneX rather than the local lab of his choice. Chances are he won't go the IGeneX route unless you've got a really good ID doc. If he does you'll be one lucky cookie!
I'm on the west coast so have used only IGeneX, but from what I understand Bowen and MDL are also reputable labs used by some docs on the east coast.
