Go to their site and either print off the forms or you can order a kit. The kit contains forms, prepaid shipping and packing for the blood sample. I ordered it and recieved it in about 2-3 days. Have any doctor sign the order. Have your blood drawn at a lab. I had mine drawn at the outpatient lab at the local hospital. Make sure the lab follows the shipping instructions...do they need to spin it, etc... Pack it up and mail it.
P.S. Don't forget to put a check in there. If you prepay it's much, much cheaper.

For almost "any lab" you put your keys in the ignition, drive yourself to the lab, plop down your arm, and viola- your blood is at the lab

Follow Lenny's advice on contactig Igenex for one of their kits.
After I did that, myeslf, I randomly called labs to see if they could draw my blood for Igenex. I hope that made sense.
I found a lab that charged my absolutely nothing, and didnt even use my insurance, to perform the testing.

Also, write on their form that you want your results FAXED to your drs. office & followed up with snail hard copy. (My results were also lost in mail or on someone's desk, and I had to pursue getting my POSITIVE results back!)
Check their preprinted form for cost amounts. Mine were $90 each for IGM & IGG, $180 total.
If they bill you it was to be $40 more!
