I suspect I may have neuro-lyme based on everything clinical I've read and observed, including real tick exposure years ago.
I also know that Igenex is one of the best for testing. But since I think my exposure was so long ago (10 years+), should I expect any positive results from even a Igenex WB?
What test do you go for if it's all buried in the CSF? Is there a test?

Some test positive, some don't. According to IGenex's lab director, if you draw blood late in the day (when we typically have our sinking spells), you increase your chances of a postive test.
Also, if you don't test positive the first time around, IGenex suggests an "antibiotic challenge", to enhance the diagnostic yield of the Western Blot, without having to go to the expense of the PCR or Urine tests. IGenex can give your doctor the details.
