You need to be sure to treat for yeast and for coinfections (bartonella, babesia, mycoplasm seem to be the cos that are most common and resistant to the Bb drugs). This made a huge difference for us in both cognitive and physical health. Sounds like you have found a lyme literate doc (LLMD)?
Check out the posts on heavy metals; you may need to treat for those.
Here are some neuro links that I've found helpful.
Neuropsychiatric Manifestations of Lyme
Lots of links to Neuropsychiatric LD info
http://www.lymeinfo.net/lymefiles.html -- look in the Symptoms file -- there are summaries of neuroborreliosis research there.

The ILADS site has some very good articles - see http://www.ilads.org/presentations.html especially by Sherr and Goldstein re neuro stuff.
I played the audio file of a discussion of Neurolyme by Sherr at this site for my husband, and we ended up listening to it a couple of times; it was somehow very comforting: http://www.lymediseaseaudio.com/sherr.htm
