I have been treated with Ketek (400mg per day) for almost 2 months. The Lyme was diagnosed in the early dissemination phase. While I made some initial progress the last week and a half have been a step backwards in symptom intensity. I was wondering if anyone had recommendations on when to push your doctor for new antibiotic and getting more aggressive in treatment (e.g. IV vs. oral).

Talk to your doctor about the change in symptoms and see what he/she thinks. It could be a herx, it could be a setback.
Are you seeing an LLMD, 400 mg/d of Ketek is a low dosage, not even the recommended dosage (800 mg/d) by the mfrg.

Yes, your dosage is half that recommended. Also I would think you should be taking another antibiotic in conjunction with the Ketek. I take Cedax (800mg daily) and Plaquenil.
Because of where you're located I suspect you might be seeing Dr. G in Basking Ridge? I am not a fan of hers.

My guess would be you are herxing. Or getting yeasty (symptoms from yeast are very similar, especially joint pain and brain fog, gi issues). Since you are on a lower dose, it may have taken a while to have enough impact on the keets to finally make you toxic.
It's way too early to be 'pushing' for anything. If you feel better than you did, you're on the right track.
