I suffer with all 3. Aren't I the lucky one. My tick wanted to go out with a bang. LOL Have to have some laughter or I would go nuts!!!!! Here is my question to you. What type of physical pain is apparent with these conditions? Second question what type of doctor would I go see for it? Is there one around? Would they judge me by my size? I have the same combination + soething else.
I can summarize a first hand account of the pain aspects:
Foot pain, bottom of feet,especially in the morning
Joint pain, sometimes excruciating, knees, ankles, wrists, finger joints
relentless headache, usually on one side of the head. nothing stops it
ear pain - transient
Lymph node pain, especially on the head/throat
Pain in the upper GI - liver/spleen, deep burning
Jabbing pain - like someone putting a nail into your bones - arms
Pain on putting ANY load on the muscles - arms, legs
Grabbing pain in joints, Knees, ankle
GI pain - cramping
That all i can remember for now.
Pretty well all of it has gone away with treatment... but does come back on relapse.

Your combo of infections is pretty common. A doctor who knows these infections will understand how they interact. Check to see if there is a support group in your area, or post in the Seeking a Doctor section, to get a good recommendation. You need someone with lots of experience.
As far as the pain, it really depends on where the infection has settled, how it will impact you. There can be joint pain, nerve pain, muscle pain, headaches, strange and bizarre sensations, auditory things, visions, you name it, it can happen.
If you are in a lot of pain, you need someone familiar to help you. Depening on wher ethe pain oroginates, different meds will be needed to help get at the cause, as well as to manage the pain itself.
