Does anyone know how Dr. H. and Dr. K. treat disseminated lyme? Do they prefer I.V. first over orals? Do they have a good system for treating out of state patients? Do they provide fast emergency on-call support?

I'm being treated with orals by Dr. H. I live in Ohio and travel to NY to see him. You do need to be able to have monthly blood work done (I go to a walk-in clinic for it).
I am very pleased with the results I am getting from his care. I think it's a good system for working with out-of-state patients.
For emergencies during office hours, they answer the phone. For off hours, there's another number to call.
For non-emergencies, they get back to you.
I feel like I have very good communication with them, which is very important as his treatment is very aggressive.

If you are talking about Dr.K in Madison, Ct, I found her not too agressive with treatment. I since changed doctors and am very happy with my new doctor. If you want I could PM with the info.

you are right about this not very aggressive.
the other DR K. is very good but only treats if positive test results.
