From reading other threads, seems like the usual dose is 500 mg.
Husband is having severe muscle pain on 750 mg. He didn't take it today, to see if backing off will help. I want to be really careful with this. We're thinking that if he feels better, he can re-start on half a pill in a couple of days.
If that is more tolerable, should we ask for 500 mg rather than 750? (He will call LLMD tomorrow, but would really like your opinions.)

Does he usually have muscle(could be nerve) pain? I took 750mg of Levaquin and had an increase of thigh and calf pain and I attributed it to herxing.

I would be really careful about the muscle pain and call your LLMD. I'm sure your LLMD told you about the risk of large tendon rupture with levaquin. I read somewhere to watch for pain on the tops of your knees, your achilles tendon and the tops of your hands.
My husband is on it too (500 mg) and I am watching him like a hawk! I do think 500 is the standard dose.

Chamade, Yes, I do think he was herxing, but what a herx it was! He is usually very stoic, but readily agreed to take a break for a day or two. Vjb, he has been herxing both his brains AND his muscles out!
His muscle pain is much better tonight, and he just seems like himself again.
Tickbattler, I didn't know about paying special attention to the spots you mentioned (above knees, top of hands, Achilles tendon), but it makes sense. Any other ways to distinguish between muscle pain and possible tendon injury? I'm also very edgy about the Levaquin.
He has promised to call LLMD tomorrow, and I will get him to ask about changing dose to 500 mg. He does want to stay with the Levaquin, but it's got to be more manageable than it's been. He's struggling to keep his job, and has to be able to work.

My husband spoke to the nurse in the LLMD's office today. He's been off the Levaquin for 2 days, and the muscle pain has decreased a lot.
He will start up tomorrow with half a pill, and if that is more tolerable, may switch to 500 mg a day.

I'm on Levaquin but only taking 500 mg a day. With that said, my doctor suggested a good magenesium supplement.
It's suppose to help with the possible risk of the tendon problems that I'm sure you read so much about on here. For me, he has me on Ionic Fizz which is a scoopful in a glass of water and tastes like a fizzy lemonade.
But magnesium is also suppose to help with pain, so I figured it was a win, win situation.

Is Ionic fizz taken once a day? And is that the actual name of the product?
I ordered the type of magnesium recommended in Dr. Burrascano's guidelines, but because of scheduling meds, husband is only able to take it once a day. That comes out to about half the recommended dose.

I took 750 mg levaquin for a 3 month period in early 2005, with 300 mg of clindamycin 4x/daily
This was for a staph infection in my foot
One month off of the levaquin, with continued clindamycin usage, and then restarted @ 500 mg levaquin for 2 additional months
DW and I put 2+2 together from those times (never dx'ed with TBI's) and realized I herxed a lot and a bunch of mystery symptoms were relieved for a spell
Long story, and this is not the time or place or thread for it
No negative sides that I or DW can recall just good old fashioned herxing

Yes, the Ionic-Fizz is a once a day type thing. I am on so many different abx, herbs, supplements, probiotics, colonix, etc...that there are too many needing to be 2 hours apart and I don't think I could fit anything else into a day so once is all I could do.
It comes from Pure Essenc Labs. I've been on SOOO many different magnesium products but when it came to liquids which he said absorb better, the taste just wasn't going down. This one clicked for me PERSONALLY.
As far as the Levequin....I keep hearing pretty much the same. The herxing is rough, but it does a heck of a job in turning symptoms around if you can tolerate it. Guess time will tell.

Well, husband has started back on 350 mg of Levaquin, and I found Ionic Fizz at Whole Foods. Onward...

My daughter's rx says 250mg 3x's a day and she is a very small, thin young woman.
What is the best brand of magnesium? Does the body absorb it very well?
