I've seen people on a WIDE variety of artemisinin protocols, including but not limited to:
Low Doses (100mg 2-3x daily) High Doses (200-400mg 2-3x daily) Continuous therapy One week on, one week off One week on, two weeks off Three weeks on, one week off Three days on, four days off
I'd like to make some sense of this with an explanation for these different approaches (assuming there is a method behind the madness, I could be wrong).
So, if your doctor put you on artemisinin, please reply with your dosage, instructions for pulsing therapy (or not), and most importantly the RATIONALE behind his/her approach.

I am just about to start artemsinin as is my son.
My dose will be 500mg three times a day, his will be 200mg twice a day. Both of us are to take it for 5 days on, 10 days off.
Dr says this is per Klinghardt protocol. I presume the dosage is based on weight.

There a doctor whose name I can't mention here who has written several books on the subject. If you PM me, I'll send you the link.
There are many studies about it. I believe, off the top of my head - that after 5 days, it builds up in your system & becomes less effective. So, if you take 500mg after 5 days the body only utilizes 100mg or something like that. So, you have to take a break. I also read that the active chemical stays in your system but it peaks about 30 minutes after you take it....
Confusing, YES... You also need to look into the various forms of artemesia. There are several & they have different properties. There was also one study that shows that Butyrate makes the artemesia more effective. It was a study geared towards cancer, though.
It's also important to take anti oxidants when you treat with artemesia. It creates alot of free radicals as it targets babesia. Some feel it's important to make sure your iron levels are normal, as well.

This thread gets into why most docs say to pulse the artemesinin:
I'd probably start off with a lower dose & work it up gradually. It can make you herx alot. I think the large dose at first is a remnant of malaria treatment. With malaria, they use a big dose right off the bat. Babesia is different.
It's my understanding that you have to take it for a while. Many malaria treatments use artemesia with some other drug. It's just taken for a short time & seems to be pretty effective in cases with (NOT) alot of other complications - I made an error here -. Babesia is different.

A useful piece of information:
Despite the common recommendation to take on an empty stomach this study found no significant difference in artemisinin pharmacokinetics with or without food. http://aac.asm.org/cgi/reprint/41/5/1069.pdf
This would make 3x daily dosing more feasible as its quite difficult to ensure your stomach is empty at precise 8 hour intervals.

One well know LLMD does 5 days on, 2 days off. This seems logical given the book I read by another well know LLMD.

Anyone seen more recent research work since that study, or a study on patients with compromised immune systems?
Just started Artemisinin 2x/day between meals; about 9 hrs apart.
Will pulse, taking it 3 wks on, then 1 wk off.
Any suggestions for best wk to take OFF given my Borrelia/Bart/Babs gang-o'bugs flare pattern? I get 3 "good days" at the same time each month.
Was amused to see that my dose was "child size" yet it still wiped me out! Glad my LLMD has been gentle to me... Smile

Buhner recommends cryptolepis now for baba.
He says if you hvent used art to go ahead and do a 30 day cycle. You'll know after 30 days whether it helps or not.
But cryptolepis is much easier on the liver and appears to have some effects on both babs and Bart. Among some other protozoans.

onbam, I'm very concerned about my hearing. Constant buzz for several weeks now and hearing on decline for several years.
Searched on Toovey as suggested and read several articles. Many were scary!

In contrast, this study clearly detects the transient quinine induced hearing loss due to temporary outer hair cell dysfunction. These results therefore support the continued use of oral artemisinine based combination therapy for uncomplicated malaria."
