That is true. There are other treatment protocols not listed on Burr. guidelines although that is a good place to start.
Some I have been looking up
Gasser protocol -300 mg roxithromycin and Bactrim DS 2 X day I think this is for lyme but people on here say Bactrim DS with a macrolide is useful for babesia/bartonella. Roxithromycin is a macrolide that crosses the blood brain barrier but it cannot be bought in the US - must order online.
Babesia LymeMD recommends using cleocin (clindamycin) and malarone when Mepron/zithro fails.
One person wrote Coarten was the most helpful drug against babesia. I don't know how it should be used for babesia but for malaria it is 6 doses over three days - four tablets each dose.
It should not be taken with grapefruit juice, zithromax or diflucan (get completely out of system).
There is also larium, quinine and other more drugs with more severe side effects.
One treatment for lyme is a certain number of days of diflucan followed by penicillin. The diflucan is supposed to drive lyme out of the cyst form. Can't remember the name of this protocol. When I took diflucan for yeast it was great for also getting rid of the lyme symptoms.

I dont know why there is confusion about doxy..
The doctors who are NOT llmds, when they see lyme (only through a rash and + test according to them) they usually throw you a script for 14-28 days of Doxycycline.. which worked wonders for me. Doxy is the stereotypical "cure" for LD according to the world.
Just throwing that out there, since you seemed confused as to why Doxy was being used.

i should add, before i am attacked, that i did not mean to say Doxy is a cure.. i was just saying that 'ducks' think Doxy is a Lyme fix all.. so its not a new thing that its being used, its been used.
for example, when mentioning lyme to a neuro one might say "OH WELL 10 DAYS OF DOXY SHOULD HAVE DONE THE TRICK, YOU JUST HAVE MIGRAINES!"

LLMDS seem to use just about anything for anything. In my experience, regular doctors use doxy for lyme. even my LLMD put me on Doxy for lyme, and it worked. I testes pos for lyme, and was only treating it with doxy, and it worked. not everyone will show improvement on it though, so its good to use different things.

I'm just a bit confused about the point of this thread.
I'm not going to ask my doctor for a script because I saw some "protocol" on the internet. And I don't think a doctor will prescribe me something because I "heard" it might work.
While the Burrascano guidelines may not have peer-reviewed studies backing them, one might argue that they're a primary source, collected from thousands of patients that he has treated, and that they're used all over the world, etc. etc.
I am fortunate to have a LLMD I trust, and I realize that not everybody does. But if I was treating without the supervision of a doctor, I would adhere very strictly to the guidelines when choosing which antibiotics to take.

Information-I just ask my LLMD-I heard that such and such is good for ____. Not every doctor can be up on everything.
Sometimes he will say-that right, actually that might be a good idea. I like to understand as much as I can about my treatment
