My only pains and aches are the muscles in the hamstring/calves and the knee and ankle/foot area - any idea why?

My pain started in my legs, and that has remained my most painful area for all these years.

My previous LLMD thought that the lyme bacteria chooses places to concentrate in the body, and that my knees are one of mine.

My current LLMD suggests using heat to induce false fevers in areas that hurt, since lyme likes lower body temperatures.

Are there ways to detox the lower extremities other than foot detox pads?

When pain is on both sides of the body it is usually a co-infection. Bartonella often causes pain in the feet and cramping in the calves.
But pain throughout your your legs can be babesia.

Borellia migrates and is more one sided. So it is more likely you have pain in one knee, not both. Then it can move to another location.

Heavy metals are called by these words because they are heavier than water and sink to the lower extremities and body regions.

Reproductive problems, female cysts, prostate problems, all go back to toxic metals and often too late. Lower jaw. Gravity.
Plantar fascitis -- gravity.
Pathogens/microorganisms go where blood flow and oxygen is scarce. They plan to survive and choose the right place.

Microorganisms do not survive in well perfused areas. They move into the toxic terrain.

Heavy metal detox should be part of any Lyme treatment, all alongside each other. It can be done at the same time. Read some of Dr. K.'s published protocols.

First bring back what is missing nutritionally, but then it is time to reduce the toxic burden and then we have a chance at the infections.

As long as your body is suffocating under the toxic metal and chemical load, healing does not happen.

Possibly read and learn to understand the "Five Levels of Healing". You can find it on the internet.

Take care.

I agree with what everyone says. It could be coinfections or metals.
I have this too, I have treated Lyme, Bart, and Babesia.

I don't have any other symptoms other than achey legs, ankles and hamstrings.

I had a period of this being my dominant symptom last year, then it went away completely.

I just came off from 2 months of brutal insomnia due to
Rifampin, then it came back.

I am starting to wonder if it is
some kind of virus, and the not sleeping activated it.

Unless you treat metals on an ongoing basis, considering possibility of KPU, with the next killing spree of antimicrobials you will free more metals. They will most likely move/sink to the same area with the least perfusion, unless you follow a good metal detox program binding metals before they can be reabsorbed and redistributed.
