Why do many LLMD not accept insurance?

For a couple of reasons:
First, LLMDs spend much more time with patients than regular doctors (hours vs a few minutes) and there is no way insurance will reimburse them for that amount of time. Think aboutthe last time you saw a regular doc. I bet the spent a max of 10-15 min with you. Insurance reimbursement is low so they need to see many patients per hour to be profitable. LLMDs would go out of business if they relied on insurance reimbursements.
Second, Lyme is controversial and most insurance companies claim it doesn't exist. There have been cases where insurance cos have sued docs for "overtreating" Lyme. So most LLMDs don't take insurance to avoid risk.
