I would choose herbals because you won't get well with just one antibiotic.

For an adult? For disseminated Lyme? An ORAL abx? (I'm going to assume Yes for all 3.)
I wish it were an easy question to answer but everyone seems to have a different abx that works best for *them*.
I know high level amoxicillun worked best for me, but a good friend did wonderfully on Biaxin (& Plaquenil... an "anti-malarial").
Avoid 'Ketek' (telithromycin). I was on it briefly & studied up on it. In my opinion it is POISON. I felt like rat poison was flowing around in me after only a day or 2. See: http://en.wikipedia.org/wiki/The_Ketek_Debacle
They supposedly came out with a variation of this "1/2-synthetic macrolide" not too long ago that has less side-effects. Whoop dee doo.

Clindamycin.. I prefer no abx.. Clindy I didn't have the horrible side effects as with the others.
I am one that can only do one at a time so guess I must try herbs ..
