The under surface of the diaphragm is innervated by the same nerves that innervate the shoulder, and irritation of that surface will usually refer pain to the shoulder on the same side of the irritation.
A swollen, congested liver might refer pain to the right shoulder because of this, but the liver has a capsule with pain nerve endings, so that same congestion should also be felt as right upper abdominal pain.
If liver function tests are normal and there is no shoulder pain or right upper abdominal pain, a more likely culprit would be something slowing impulses in the phrenic nerve, the nerve that controls diaphragm muscle contraction.
