My mom sent me this pic of her neighbors 18month old. He's had this for about 1 week. No other symptoms that I am aware of.

Looks like a bulls-eye rash to me. It's possible that it's NOT one, but if it was on one of my children, I would have them immediately put on antibiotics (in a young child it would probably be 4-6 weeks of amoxicillan).

- Sure looks like a bulls eye to me, too - especially with the raised and peeling center.
I'd get the kid to a LLMD for assessment. I so hope there is another explanation, though (and there may be but it is highly suspect).
Maybe they could forward the photo first with detail as it if a tick had been seen &/or removed and any symptoms. -

My daughter had a similar looking rash that I was told was from a mosquito bite. I didn't want to take chances so I treated her.

Okay...I got some more info and I told her that she needs to have him treated. They were visiting at the grandparents house and he was outside playing in the grass and the pool all day.
This rash was noticed two days later and mom says he has been cranky all week. Appears to be in pain. No fever.
I told her if it were me, I wouldn't take any chances either and would do what was needed to get him on abx.

I wonder if he has a headache...that would make him irritable.
She could try giving him a dose of children's tylenol, and see if he acts better. If he gets worse again when it wears off, that may be part of the problem.
Also, she should make sure he's getting enough fluids.
