In a case like this, I would personally use a cortisone suppository (Anucort). I'm sure therhe are plenty of folks here that will advise directly against it. I had a horrible external hemmoroid, and a bad internal one too. I used 1 suppository and then used something called Zenmed Ziro, which is a fantastic natural ointment.
That one suppository cleared me up very quickly and I haven't had a problem since. It's been 5 months.
One lovely side story about this - The Ins would not pay for this, so doc re prescribed different brands and strengths 3 times. By the time I buckled and paid for it, everyone in the pharamacy (which is mostly young girls) had a conversation with me about my Anucort, or Anusol, or whatever butthole med my ins was rejecting this time. It was pretty humiliating. When I eventually bought it, the one girl asked the woman behind the work counter if they had my Anucort ready. This is in a crowded Supermarket, with people behind me in line! I was wondering if they planned on making me as uncomfortable as possible. But really who cares. I should be happy there were so many people working on my butthole.

For me, taking metamucil helps to reduce pressure when you do have to go. So healing will be faster.
Preparation H works well.
Not fun, btw.

Believe it or not, coconut oil has been recommended for this. It might provide some relief in addition to the above mentioned. Good luck!

I tried all of the above mentioned things, and they were soothing, but the hemmoroid was so popped out that nothing was helping. I tried every OTC product available, and the only things that gave me a permanent solution were the Anucort along with Ziro.
Good luck to you, I know how terrible this can be for sure.

I would never admit to having butt problems, but this is my Dr's formula:
B-6 Rutin Hesperidin
These are taken orally. They dry up the hemorrhoid. I've never had an external one though.
I bought these online from Swanson's. Very cheap. Take them less than 10 days.
They are pretty drying so drink tons of water.
You might try calling the Dr's office and speaking to the nurse??? She may just ask the Dr for a script and call it in for you. A little less embarrassing!

Need to call the nurse at your doctor's office about this...sometimes, an outpatient office procedure is needed to deal with this severe a hemorrhoid.
For non-thrombosed hemorrhoids, I like a home-made suppository containing coconut oil (organic) and a few drops of essential oil of Cypress mixed into the coconut oil, then poured into a suppository form (can buy the suppository forms at a compounding pharmacy).

I have been soaking in epsom salts and applying Prep-H with no relief, but it will probably take a few days.
Guess I will have to make it through the weekend.
These babies are bad news! This is a first for me...can't believe how painful!

PLEASE go to your doctor! The prescription creams are SO much better than the OTC ones. And eat lots of fiber and the Metamucil is a good idea. You don't want to irritate it any further. It's been a long time but I think there was something I soaked in too. Sorry - I can't remember more details.
Strongly advise against any medical procedures on this (unless absolutely medically necessary). I had that done years ago and it was, hands down, the most painful thing I have ever experienced. And I have been through migraines, childbirth, and several surgeries so my pain threshold is not low. Much better to get the prescription creams and use that to let it go down.
Until then, recommend cortisone over Prep H. But again, if you are in a lot of pain, call and get the prescription ones - don't suffer needlessly!

I am scared to go for medical help because I am terrified they will recommend steroids.
Will it be ok to use a steroid suppository or will it make lyme worse?
I am on abx.

You almost have to have the steroid suppositories, in my non-medical opinion.
They should not make the Lyme worse as long as you are on abx.
Others may disagree with me, but that's OK too.

Any doc who recommends steroids for a thrombosed hemorrhoid is committing malpractice. Large ones should be excised, smaller ones will shrink and disappear over time. Some docs will numb them, and simply incise them to pop the clot out, which will make the pain go away instantly, but the vein can seal back over and the thrombosis may reoccur. A musculoskeletal adjustment may help with venous and lymphatic return from the congested area and also alleviate symptoms.

Had no choice. MD would not lance...said they don't normally recommend that. Anyway, prescribed Anucort-HC suppositories although I stressed my concern with steroids. He said it was only a small dose of steroids and stayed local in the rectal area....yeah right.
