I am seeing my 3rd or 4th LLMD on Monday. I have been in treatment since spring 2007. Been on many many rx combinations. Any suggestions for how to share my history in a concise manner?

Make a simple chronological list of ABX combos that yoo have been on

For Example:

June 2012-Present: IV Rocephin Mepron, Zith, Bactrim
April 2012-June 2012: IV Rocephin, Zith, Mepron

etc.

Include dosages

then list all current supplements and herbals.
Dates are not as important. Maybe Group them as current and past so they know what you have tried.

List all relevant test dates and results.

That should be about two concise pages
