I have started Rifampin (for about a week), 600 mgs per day. My toes, especially the two big toes, used to give me a lot of problems. Hadn't had problems with painful toes in awhile.

It isn't in the big joint at the bottom of the toe, it is the first joint. Gout usually hits the bigger joint towards the bottom of the toe. It isn't swollen, isn't red, but it is so tender and painful.

Now that I have started Rifampin, my toes are back to hurting. Very painful big toes, can barely walk. Is this coincidence, or could this be a herx.

Glad your toes are better. I had a similar experience, with my first Herx, an intense pain settled into the tops of my feet. First one then the other. Other symptoms changed and got better, but the pain in my feet stayed until just recently I was treated with Bactrim for an ear infection. My foot pain has lessened by 70% miraculously for the first time in almost 3 years. So I suspect Bart too. Hope it stays better for you.

It isn't quite as bad as yesterday, but it is still pretty bad in one toe. My pain is in the top joint of the toe.

I had these pains when I first about a year ago when I first started to get my treatment. It went away for a long time and now it is back.

I used to tell myself the bugs were hiding at the point farthest away when I got that toe pain. It typically would move on to another spot or other foot or be just gone the next day. Had this reaction just using Diflucan as well, so I would say herxing.
