I've had this rash on and off for months that keeps coming back in the exact same spot. It's sort-of like a poison ivy rash, yet I had no exposure to poison ivy (and if I did, I'm highly allergic, so I would have it in more spots than this tiny area of my low back). Thoughts?

Does the rash have a shape? If so, what shape? How large of an area does it cover?
Does it itch? Is it raised, warm?
How long ago did you get lyme?

- check ingredient safety on all kinds of personal care and household products - do not be fooled by "natural" or even "organic" on labels as they may not mean exactly what you think they should mean. Read the full list and study ingredients.
Even Seven Generations hand dish soap contains that ingredient in the post above. When I stopped using that, my hands got much better. -

You say it's a bit like a poison ivy rash. Probably best to keep any oozing of it under a gauze pad -- and use a separate towel (pat with a paper towel) to dry off that area so it's not as likely to spread around if it's a kind that could.
Some Herpes strains can also show up anywhere on the body, including low back. Various infections can follow nerve "highways" so to speak.
Photos in natural light of changes. A doctor really should take a look if its recurring. -

Very small area, at most about 1 inch by 1 inch, no specific shape. Raised, itchy, but not painful. It will start to dry up and look better, then recurs weeks later. It's usual state is very much like poison ivy that is starting to dry up.
I see a dermatologist (plantar warts that I'm having frozen every 3-4 weeks) and the ironic thing is that every time I go, the stupid rash has gone dormant and doesn't show up. (The rash definitely isn't a wart)
Not sure how long I have had Lyme, I've been worse than a normal state for 3 years, so maybe 3 years. I did take in a stray cat last fall who had fleas and I have been in worse health since taking him in, so I'm worried that maybe I contracted Bartonella from the fleas (the rash started earlier this year, several months after taking him in). It's definitely not streaks like the bartonella rashes I see online.

Or could indicate a nutrient deficiency, too.
Oh, looking at your posting history, this is the only indication of Rx that pops out:

But, then that would not be a LLMD treating you, most likely. -

- Lower back. Label from clothing? Either the physical irritation of the label fabric or dyes, perhaps. Even no-label clothing often has a rubbery stamp that could be irritating.
Dye or chemical treatment from actual clothing other than tag, too, would be a consideration and that part of your body might just be the most tender point to erupt. -

Yup, treating, but no LLMD. Using a naturopath in my area - he's actually pretty knowledgeable about Lyme and chronic infection and is far cheaper than the one LLMD in the area, who is over $700 just to walk in the door and makes you pay a non-refundable fee up front to schedule an appointment that is months in the future. Part of the approach for us has been to treat using a broad method so I don't have to pay thousands of dollars for tests just to find out what I have.

BTW, I'm taking minocycline also. I was thinking maybe mino could cause it, but I have taken it for a while and didn't have this reaction until fairly recently.

- Your ND should assess this. Be sure to take some photos in case it's not visible the day of the appt. But hope they can see it up close.
Any kind of reaction to any Rx, OTC, herb, food - even to personal care products - can just start up at anytime, even if prior use had been all dandy.
If it might be an infection of sorts, though, just be sure to keep it covered with clean gauze when it's erupting.
Could be fungal in nature, too.
