I am suffering from chronic Lyme and have also been diagnosed with various other conditions that are making my treatment more complex such as POTS, thyroid disease, mold toxicity, food sensitivities and more. I need a doctor who is Lyme literature AND can deal with my numerous collateral conditions. Although I live in Orlando, FL I would be willing to use any doctor in the US who fits my criteria.

Welcome to Lymenet! PM sent for various states.
You need to be evaluated and treated by a Lyme-literate doctor (LLMD). Non LLMDs have no clue about this horrible disease or its complex treatment!
A LLMD is one who has treated Lyme disease and the co-infections which come with it for many years and has gotten patients well. A good one will follow Dr. B's Guidelines, the "gold standard" for Lyme treatment.
Unfortunately, LLMDs are far and few between. You need to go where they are. At least half of all Lyme patients travel out-of-state to get proper treatment.
Also most LLMDs do not accept insurance due to the politics surrounding this horrible disease. Read poster TF's explanation, "Why Lyme Doctors Don't Take Insurance":
Read the books written by the top LLMD, Dr. H, titled, "Why Can't I Get Better?" and "How Can I Get Better?". They are an excellent source of information.
Also read "Cure Unknown" by Pamela Weintraub. Get it at your local library or buy them used online.
Btw - I know you are new to Lymenet, but you should not use your real name (if you are) as your username, because this is a public forum with all sorts of people on it. Read this link about it:
