Willing to travel outside of NJ for a very good one, but preferably not too terribly far outside of NJ. I'm VERY close to Philadelphia if that has any bearing on finding a very good LLMD. Hoping that they also accept medicare, but I don't know many places these days who don't take Medicare as it seems pretty universally accepted. If there are truly no very good LLMD in NJ then I will be willing to see what is available out of state.

Lymeboy was saying in another thread that there are certain doctors in NJ that must be absolutely avoided, which ones was he speaking of? I want to make sure to cross those off the list as avoids.

Unfortunately, the "very good" Lyme-literate doctors (LLMDs) don't take insurance at all.
The top LLMD, Dr. H, has written a new book entitled, Why Can't I Get Better?" It is an excellent source of information.
Read "Cure Unknown" by Pamela Weintraub. Get it at your local library or buy it used on Amazon.
