Dr. Joe Burrascano recently closed his medical practice. If any of
you are former patients, or are caring for a former patient, and are
in need of a copy of their medical record, this is whom to contact:

Iron Mountain Records Management

Route 9W South, Port Ewen, NY 12466

Phone 914-662-2024

Mention Client Number LH404

Please be advised that upon the receipt of a request for a medical
record by a former patient by Iron Mountain Records Management:

An Iron Mountain Records Management Customer Service Representative
will contact the requestor.

A summary of charges for the specific medical record will be sent to
the requestor.

A Release of Information Authorization form will be sent to the
requestor to fill out and sign.

Once payment has been received along with the signed Release of
Information form, Iron Mountain Records Management will release and
ship the requested medical record.

Notices will be mailed out, but it may be possible that interested
parties do not receive the mailing, so that is why I placed this info
on the web.

Please feel free to forward this message as appropriate.

nan, thanks for posting. i was wanderng about that since my country dr. from 50 years ago have records i would have loved to have gotten when she retired 30 yrs. ago. she recently died.


nan, do you know? are they planning on charging so much money PER SHEET? that would be extremely expensive for patients like me whose files are 3-4 inches think.

no, i didn't go to dr. b., just a comparison!

Betty, I believe they are entitled to charge a reasonable fee for copies. You will have to call and ask.

too bad you can't take your own paper in and be charged so much for the toner used 5 cents.

campus copier stores charge 2 cents a page!

yes, i could NOT afford 75 cents for my 300-400 pages in my 2 files at clinic! [Frown] [group hug]

when i did disability hearing, i paid clinic $15 flat charge for 15 years worth!

they should have something affordable for you folks; i feel for you.

FYI, I GET A COPY OF EACH DRS. MEDICAL REPORT, XRAYS, BLOOD LABS, ETC. as they occur; NO cost since i paid the lump sum in the past.

will be charged again in future but as long as it for several years with MANY PAGES it won't be bad! [Wink]

75 cents a page doesn't sound reasonable to me.

for going forward or newbies, if you ask for your records from each visit for the prior visit, or small amounts of paper each time from the receptionist - it's usually free - but allot to keep track of.

is there anywhere that puts a limit or cap on reasonable fees when you need the whole file?
the records are your property, after all.

would this be something EACH state's insurance commissioner would know or act upon?

to be 75 cents is outragious! that's why i voiced my opinion above. [cussing]

again, to get SSDI, DDS folks pay a FLAT SUM, not much for the persons records for 6-12 months only! we'd be better off going thru them to get records than pay that much money!!!

75cents dont forget someone is standing there making those copies.They have to be paid for there time.

seriously, i was a secretary for 25 years; they have AUTOMATIC feeders that do 2-sided copies and could be done in 30-60 minutes tops.

put them in; hit start button and walk away until you hear it not working...

check for out of paper or a paper jammed up.

so i could see $20 tops for person feeding the automatic copier; but that's it plus paper expenses!

sorry, but it's robbery in my secretary eyes! we've paid dearly OUT OF POCKET for our LLMD visits, tests, etc. again, my 4 cents worth due to inflaction. [Wink]
