I've also gotten things through Fallon's Compounding Pharmacy in New York (can't remember which town) but I can't find a website for them. I know it's a pretty well known compounding pharmacy so maybe someone else has more info to add about it.

I haven't used Iowa's new compounding pharmacy yet. They can't make B-12 unless they have at least 12 patients needing it.
I'm now using all the below except Iowa.
IOWA PARKVIEW PHARMACY in NEVADA, IOWA 50201 515.382.2134
MISSOURI O'Brien Pharmacy, Kasnsas City, MO 64111 1.800.627.4360 fax 1.816.531.4033
NEVADA Kronos Pharmacy, Las Vegas, NV 89103.1059 1.800.723.7455
my nystacin is $86 plus #21 OVERNIGHT s/h = $107 TOTAL
WISCONSIN THE PRESCRIPTION CENTER, LaCrosse, WI 54601 1.800.203.9066
PS - Wisc., I get my B-12, methylcobalmin from $75 plus $2 postage = $77 total

I mostly use
Wellness Pharmacy in Birmingham, Alabama (800)227-2627
They have a Pharmacist out on leave for lyme treatment.
They are so helpful and always willing to talk to you and/or your Dr.

Clark's Pharmacy in Bellevue, Washington 425-881-0222
I used this one for several meds while I was there a few weeks ago.
They were very friendly and fast, I was on my way to the airport.

For those in Oregon, it's about 25 mins N of Salem and about 30 mins S of Portland.
Nice people and easy to work with! JavaBeing

Medicap Phamacy Tom Zeigler Ahburn, VA, Northern VA
(703) 858-9616
Family owned business. Does compounding, prescription meds, supplements, and homeapthy, and medical supplies. Will ship. Delivers free within area.
I know this phone number by heart as he gets to know his customers and is personally involved in keeping track of every single prescription med, supplement, etc. we are taking to detect any possible interactions. Can't trust big name pharmacies to do this for myself and kids.
