There are lots of obstacles to overcome in order to move back to CA.
But, some things to consider are:
A town close to my LLMD (Dr. H)
Public transportation that is power chair accessible.
Low Crime, good air quality. I prefer small towns.
Need nature close by.
Mild temps in the summer. I do not do heat well.
I like having a bit of snow in the winter.
I also am not sure if I could handle living on the coast at this time either.
Lived near the beach before lyme but not so sure I could handle the dampness now. Miss the beach though.
Looking for suggestions of towns to look into.

Note: i have not seen Dr. H at his main office...only the malibu office.
So, do not know anything about that area with the exception of being in SF a few times at UCSF Medical Center when I was looking for answers.
I am a Southern CA native. Southern CA is too crowded for my liking.

I'm a CA native also, (Monterey/Fort Ord) but haven't been back since the 60's.
I don't do heat very well either and am dying down here in the south.

I too can't help since I'm on the other coast, but I'm definitely going to read any suggestions as you get them because it really sounds like you have pegged what so many of us would find as an ideal place.
Good luck in your search and please let me know when you find it. I may like to move in right next door.

kam-how close or far away are you wanting to be from your LLMD?
His office is on the peninsula. There's lots of nice areas but some are much more pricey.
There's also the east bay which includes Berkeley, Oakland, Albany, Alameda, Pleasanton but it's about 45 minutes from his office.

Berkeley and Oakland have way too much crime. Before I get hate mail I was born in Berkeley and it is just my opinion after having worked there too. Oakland gets even more crime then Berkeley.
If you do not like the heat you might like foggy Half Moon Bay. Close to the beach and your MD.
I am all for the heat myself and I have been swimming in a bay around here lately and live 11 minutes from a beach where I can swim. If it was not for the ticks and fog and winter weather it would be close to perfect.

While I agree that Oakland has way too much crime as they are short on police officers (not a good thing) I disagree about Berkeley. Some parts have more crime than others but if I had a choice to live in the Bay Area other than where I live it would be Berkeley or Albany.
