How I downsized when I went from earning $50,000 a year to living on SSI due to health going south.
1. Sold car
2. Signed up for several apartments that went by your income in determining what your rent would be.
3. Moved from house to low income apartment.
(People helped me have a yard sale to help with down sizing and donated other things to thrifts shops each time I moved and didn't wanted to pack as little as possible)
4. Tried to use food banks but later learned they were actually making me worse instead of better.
As I learned I need to be on a no sugar/no gluten food intake.
Food is still a issue.
WE could do a post on ideas for that alone as far as stretching the food dollar and eating healthy and having easy to prepare meals.
5. My doc has been great at doing the best he can with the resources I have.
6. Shop at thrift shops and just wait until whatever I need shows up at a thrift shop or yard sale.
7. I also find I am easily influenced and quickly go back to thinking I have a budget like I did when I am around some normal" people.
So, I need to be careful who I am around or I find myself purchasing things that are not in the budget.
8. I signed up for low income utilities.
That is all I can think of right now.

I think it is great you are still able to work a bit.
Hoping that you improve enough to work more with time and treatment.
