This is an important survey being conducted by Dr. H. and SUNY New Paltz. Lyme patients and others who are not infected are urged to participate. Please participate and then forward. Your responses matter!

The purpose of this research is to study outdoor activity and Lyme disease. All individuals 18 years and older may participate in this survey.

All data will be confidential and anonymous so please be as honest as possible. The survey should take about 15 minutes to complete. Your participation is completely voluntary; you may stop participating at any time.

This study has been approved by the Human Research Ethics Board of the State University of New York at New Paltz.

I'm not sure at all that it's appropriate for those who are ill to complete this as it seems intended to be for those who are not ill - or at least those who are still active enough to be able to engage in the activities on the first page.

It asks all kinds of questions about outdoor & physical activities one engages in. Seems this would be to determine risk factor for tick exposure.

If this is completed by many who are ill and unable to do most / all of those, it's not going to be accurate for risk factor.

Page two is a good symptom asking list but as someone homebound, I'm probably not in the demographic they seek for this. Not active in the out of doors at all (sigh). USED TO BE, though but it asks of present time.

The present time "requirement" throws this off, I think.

The psych questions toward the end struck me as troublesome.

Agreed, the last section is completely irrelevant to any survey regarding patients with Lyme Disease.
