You don't have to have Lyme disease to answer the Western Wisconsin Lyme Action Group's Lyme and Associated Diseases Knowledge Questionnaire. The group is trying to find out what people know (and don't know about) insect-borne diseases.

The group hopes to tally the questionnaire's answers and present their findings to legislators and public health sometime in the new year.

Questionnaires will only be accepted until December 31st.

You do not have to give your name. The group is mainly interested in the county the respondent(s) live in, in Wisconsin.

If you would like to fill out a questionnaire or help get them out to people, please post here or send me a PM.

The more questionnaires that get out and come back, the stronger the group's data will be.
