Found this on a site I was on, might help everyone...

Hi. My name is ******** and I have been suffering from chronic Lyme disease for at least 7 years but have only been diagnosed this past July. I have developed a free resource for people to enter their symptoms online and they can request the reports to be emailed to them. The symptoms are rated from 0 to 10 and the weekly report color-codes these results so you can easily see trends. If you would like to try this out the website is http://lyme.coconia.net/lymesurvey/.

It appears to take several days (?!) to generate report, but survey is nice and short....not much room for 'fringe' symptoms but covers the basics well.
