I was bit by a tick in late June 2007. Diagnosed with Lyme a few months later but started on Amoxicillin right away (didn't have a good response to Doxy). I felt better initially, then worse again, then added Flagyl, which helped me sail through half of August until I started going down really hard.
By September I was sure I was going to die, and my doctor switched me to Cipro. Then I had a miraculous improvement in some symptoms but had to stop due to tendon pain and elevated liver enzymes (after about 6 weeks).
Soon after that, I started working with an LLMD who said my positive clinical response to Cipro indicated bartonella. I have had positive Lyme tests but still not for bart. I'm testing with Fry labs soon.
Due to ongoing elevated liver enzymes, I was off of antibiotics and just doing herbs for awhile, then went back on Amoxicillin at the beginning of January. And whoah, I feel terrible. My liver enzymes are still elevated.
Could it be a month-long Herx from the Amox? Shouldn't I be done herxing by now? When can I start feeling a little bit better?

Have your LLMD check your vitamin D level, and if low supplement gently but get off all antibiotics for a time. Liver enzyme elevations need to be investigated for the viral hepatidities, and you should not assume that they are just the result of the antibiotics or the Lyme, they well could be, but those aren't the only possible causes. Good luck.
