Have you heard of a Lyme manual written by a mom in NJ (or MD?)...on how to deal with the school system on behalf of your children with Lyme disease?
Dr. VTS mentioned this manual. She said there should be a link from the "Teacher's observations" that antibiotics helped a 3rd grader's school performance...an article posted on her website.
Unfortunately I couldn't find an author or other link to this information.

I'm not sure if I have what you are looking for, but your question did make me think of the Lyme Times, Fall, 2003 edition:
Begin on page 11 for an overview. There's an article on Lyme pain in children from Dr. VS, and an excellent article on school accommodations from clinical social worker SB.

This link has excellent information regarding Lyme disease in children, educating students with Lyme, children with Lyme pain, and other stuff.
I left a message for SB in hopes she will know of the manual I'm looking for.

This issue (No. 35) is not "THE Children's Issue" of The Lyme Times that is said to be an excellent resource for parents of children with Lyme disease.
