Remember to ask your new LLMD about the best way to contact him/her with questions. We were surprised to learn that my LLMD prefers faxes - and we get phone calls back every time.

let's see --
1. family history with medical background 2. copies of all tests 3. list of prescriptions 4. allergy listing 5. most important, narrative on when you think you got lyme with symptom listing 6. list of supplements you're taking 7. listing of all doctors you see with telephone numbers, etc., like pcp, gastro, etc.
then keep a listing of those questions for yourself. i had so many i can't remember...
