Seeing Dr. S in Towson, Md. Quick background - have been on levaquin since April which was off and on due to early tendon issues. I was able to work up to a full daily dose of 500 mgs in mid May.
Also taking 100 mgs of mino (was taking 200 mgs but had bad ear ringing). I kind of plateaued on the levaquin/mino combo and hadn't been herxing (though my symptoms have seemed lighter but still present).
Two weeks ago my doc had me add Bactrim 2x daily and I have been herxing like crazy since the first day.
I will continue the levaquin/mino/bactrim mix for the next two weeks. After that he wants me to take a day or two off levaquin and substitute zithromax for it for the next two weeks. After that two weeks go back to levaquin. He cited Dr. J's in Carolina?'s work on keeping the bugs off balance with different and varying combos.
So I'm trying to hang in there and remain positive.

Glad to see you have a plan.
1. you might ask him if it would be okay to take B-6 with the mino as that might help offset the tinnitus (ear ringing). It has been helpful to prevent some of the vertigo often associated with mino and it may help lower the tinnitus, too.
Magnesium might also be a good help, but be sure to ask first and it works best with B-6, too.

It is very good to hear that you have a doctor who is familar with Dr. J's hard work ( although he has since moved from N.C.). That means you have a doctor who is awake and hungry for knowledge from others' successes in the field. A good sign.

This may be overwhelming to you right now. If so, you might want to file these links away in case you want to learn more about tinnitus and some of the things that help others.
Some over the counter products can be a trigger as can aspartame/nutrasweet/Equal and MSG (in all it's names. you can google for those other names).
Liver support will help, too, as decreasing the toxic load helps the ears and the brain settle down.
Thanks for the reply. I'm currently not taking a b6 vitamin but my doc did recommend a supplement he developed called Qnique which has b12 in it. Supplement details posted below and you can get it here
I'm also taking about 1,000 mgs of magnesium to help with the levaquin tendon issues.
