I assume you have tried other pain medications?
Hopefully somebody with expeience with these medications will speak up.

My LLMD was once a medical manager of a detox clinic. He WILL NOT prescribe methadone.
Just an FYI...my mother had chronic back pain and was on EVERY medication you can legally get.
When she was in her final days in hospice, they had a hard time getting her pain under control because the methadone turns off receptors to other pain medications.
They were able to get it semi under control with high..high doses of morphine. But, we are talking terminal stage levels of morphine.
My daughter recently got off vicodin and percocet because an hour after she took it, the pain came back twice as bad. Rebound pain. Knowing that she was heading in the direction of her grandmother, she opted to do without.
She now takes Tramadol and ibuprofen. It's managable. She hopes to get off of all pain meds.
I know the pain is unbearable, but be careful.

Methadone is really a last resort. It has a lot of side effects not even really related to the opioid itself.
Hydromorphone is Dilaudid if I remember correctly. The IV form I had in the hospital a few weeks ago, and it is quite powerful and helpful, but also short acting (only 2 hours or so at most. The PO form lasts longer and if you can get it, I think it is a great drug as it only requires a very small dose for tons of potency.
I currently take OxyContin, and took vicodin/percocet in the past. For whatever reason (there are several), most LLMDs do not prescribe narcotic pain medication. Ask if your LLMD knows a pain specialist who will and is friendly to lyme patients.
I may very well be switching to Dilaudid, Opana, or MS-Contin soon. These are "high tier" opioids, just like those you described, but can provide long acting pain relief.
