Go back and try to get more extended treatment.
Keep a symptom log with anything of import including vibrations, pain, mood, etc. These can often lead to dx of a coinfection that may have been missed early.
Keep her off of all sugar as much as possible- the spiros love it.
Call Dr. J and set up an appointment for later - the wait list is 4 months or so and hopefully you'll be cancelling it anyway.

I would definitely start treatment and like catskill said, track the symptoms daily. I do with my 3 young children. If your daughter had bells palsy, to me that would indicate that the lyme had become neuro and that 3 weeks would not be a long enough treatment time.
I would not wait. This is what happened with all 3 of my kids. First, the pains and strange sensations would be once a week. Then twice a week. Then it was daily and pretty soon they were quite sick. They are much better after a year of treatment so far.
I would also treat for coinfections, because as Dr. B says in his guidelines, for those with chronic lyme, the coinfections are "universal". Everyone with chronic lyme has them. My kids have tested positive for several of them.

My now almost 7 year old (infected when he was 4) has chronic lyme with many complications & does not have co-infections. We have been with Dr. J since May....
