I see nothing too meaningful or abnormal with these results. Am I missing something? The AST appears to be heading where mine is. [Frown] Oh, I'm sorry I missed the Myoglobin lab. Thst is crazy abnormal. [Frown] What's that mean?

How's he been feeling the last few days?

Couldn't get the chart above to line up very well, but this is what hubby discussed with his LLMD consultant.

The conclusion is that hubby had a major hemolysis event (die-off of red blood cells) from a babesia herx when he ran the high fever and was in the ER and hospital.

While in the hotel room 2 nights ago hubby had a mild fever of 100.5 and the next morning his PICC line had blood in it and I couldn't flush it.

He had the PICC line replaced today and they even put it back in the same arm. So as far as anyone knows the PICC line was not the problem. The technician thinks the valve in the PICC line malfunctioned due to age -- over a year old.

The new plan is to slow down babs treatment slightly. No more flagyl for now.

But will continue on malarone (decreased from 4 to 3 pills daily). Will also continue on the high dose IV clindamycin and qualaquin -- but instead of 10 days on and 10 days off will be on the combo for 21 days before taking a break.

We are not sure how many cycles will be done at this point. Hubby made it thru 2 complete 10 day cycles and was on day 9 of his 3rd 10 day cycle when he stopped due to the fever. Was off babesia meds for 12 days except for he continued the IV clindamycin.

We plan to do some additional babesia testing. Will report results when available. The LLMD asked if the hospital did a bloodslide when he had the fever and hubby just laughed.

Neuro symptoms improving slightly since hubby is home and back on a more normal routine and discontinued all the extra meds which just complicated the picture.

I think it is going to be a very long 3 weeks.

I got some kind of G.I. virus while on our trip to the LLMD and was up 6 times last night with diarrhea. So far hubby hasn't caught my virus and I sure hope it stays that way.

The drop in RBC, hemoglobin, hematocrit and platelets is pretty significant from the first ER visit on 3/25 to the 2nd ER visit the same day and the hospital numbers on 3/26. Platelets at 149 would have been low and out of normal range. Also rapid increase in RDW indicates destruction of red blood cells.

The myoglobin could reflect either muscle or blood cell destruction and would be expected to rise with a high fever. But this number was measured within an hour of the initial temperature spike which is quicker than I think it takes for muscle destruction to show up.

As I reported before blood cultures were negative so the only explanation anyone has at this point is a babesia herx. The LLMD was pretty sure that a blood smear or bloodslide would have been posiitve if one had been done.

Wow, I'm so glad he is feeling better. Sorry to hear that you are not feeling well.

I'm very interested to hear how this protocol goes for your hubby. I am suppose to start Clindamycin and Malarone but I'm worried about C-Diff.

How much Flagyl was he taking?

He was doing 500 mg of IV Flagyl once daily. Started it during his off cycle on the quinine. Initially was running fevers of 100.5 which lasted for a couple of hours every couple of days. Had been on the IV flagyl for 16 days when he had the very high fever (105.8 rectally) per the ER.

Hubby had done several months of tindamax when he was on the IV Rocephin and IV Zithromax, but had never done much flagyl before.

There is a journal article which says 3 out of 4 dogs with babesia gibsoni were cured with the combo of flagyl, clindamycin and doxycycline. Had not gotten around to it, but our plans were to add in minocycline to his combo at some point in the future.

Hubby really wanted to retry the flagyl because even though it was causing fevers it made his head feel better -- he says most IV antibiotics including the clindamycin do not get into the back of his head where his major problem is. He also said that the flagyl helped his neuropathy in his left arm. His neuropathy is very minor and comes and goes, but he actually felt like the flagyl was getting to some pathogen that was irritating his nerves in that arm.

I think the thing that is bothering him the most now is the major increase in fatigue. He tried walking and made it 1/4 mile but it was not easy. Prior to the fever he was walking 1 1/2 miles most days.

I had terrible die-off from clindamycin/quinine .. really bad. .. So I can relate to his awful herx. I ended up in the hospital twice in 9 days because of my babs herx. But I didnt' have high fever.

Bea, glad to hear your hubby is doing better. You both amaze me, in that you have the ability to get back up from this, and try again. Sheer determination will get your hubby better.

I couldn't handle the Clindy Quinine combo, and I can handle a whole lot of abx at one time with no problem. One Quinine pill, and I was really sick with flu-like symptoms, achy, eye irritation, and a red bumpy rash. It lasted three days, even after stopping the meds. I have no problem with IV Clindy.
