Please consider an SSRI antidepressant. I too had disabling anxiety for years before I even knew I had Lyme. Was put on Paxil. It was a miracle drug for me. It was like flipping a switch, felt NORMAL for the first time in my life. No anxiety, no panic, no depression, and I had a very stressful job at the time. I remain on Paxil to this day. Don't suffer needlessly. This drug has NO side-effects for me. You do not feel "sedated" like with valium, you feel NORMAL.
I know others are against antidepressants. I am not. They can be life-saving for some people.

You have a lot on your plate right now. Lyme by itself is hard, then to have all these other things would be scary.
Starting with a new doc is a biggie too. Maybe if you start your list of questions it will make you feel more in charge of your medical care. Just an idea.
You are a courageous person, even if you don't feel like it. To go thru all the stuff in the past and now, you have more courage than you know.
hugs.
